The staff members and healthcare professionals of this institution may request you to provide personal data, including but not limited to your name, gender, date of birth, type and number of identification document, contact details, email address, personal and contact information of your emergency contact person and/or relatives, credit card or debit card account and billing details, insurance information, and information relating to your health condition.

If you purchase or use prescription medication, such medication must be assessed and prescribed by a Hong Kong registered doctor based on your personal health condition, medical history and relevant medical needs. You are therefore required to provide accurate, complete and up-to-date personal data and health-related information for the purposes of medical consultation, review, assessment and prescription. If you fail to provide the required information, or if the information provided is inaccurate, incomplete or insufficient for the doctor to make a professional medical assessment, this institution and/or the relevant healthcare professionals may be unable to provide prescription medication services, prescription arrangements, delivery or pickup arrangements to you.
如閣下購買或使用非處方藥物，閣下可自行選擇是否提供個人資料。然而，如閣下選擇提供相關資料，本機構可按實際需要使用該等資料，以協助處理訂單、安排送貨或自取、提供客戶服務、跟進查詢或投訴，以及提升服務質素。
If you purchase or use non-prescription medication, you may choose whether to provide your personal data. However, if you choose to provide such information, this institution may use the relevant data where necessary to process your order, arrange delivery or pickup, provide customer service, follow up on enquiries or complaints, and improve service quality.
本機構及其授權人員亦可能按需要向任何適當第三方查詢與閣下有關的醫療記錄及／或相關資料，以用於與閣下健康護理或治療直接相關的目的，包括但不限於核實閣下身份、安排服務預約及登記、就服務安排與閣下聯絡、處理帳單及／或保險索償事宜、調查及處理查詢或投訴、醫學統計及審核。
向本機構提供個人資料一般屬自願性質；惟就處方藥物服務而言，由於註冊醫生必須根據閣下提供的個人及健康資料進行專業評估及處方判斷，相關資料屬處理該等服務所必需。所有收集所得的個人資料可能會於本機構的醫療系統內互通及使用，以便提供適時、有效及安全的醫療服務。閣下須確保所提供的個人資料準確、完整及最新，否則可能影響本機構及／或相關醫護人員為閣下提供健康護理、醫療評估、處方或相關服務。
This institution and its authorised personnel may also, where necessary, make enquiries with any appropriate third party concerning your medical records and/or related information for purposes directly related to your healthcare or treatment. Such purposes include, without limitation, verifying your identity, arranging service appointments and registration, communicating with you regarding service arrangements, handling billing and/or insurance claim matters, investigating and handling enquiries or complaints, and conducting medical statistics and audits.
The provision of personal data to this institution is generally voluntary. However, for prescription medication services, as a registered doctor must conduct a professional assessment and make a prescription decision based on the personal and health-related information provided by you, such information is necessary for the processing of those services. All personal data collected may be shared and used within this institution’s medical system to facilitate the timely, efficient and safe delivery of healthcare services. You are required to ensure that all personal data provided is accurate, complete and up to date. Failure to do so may affect this institution’s and/or the relevant healthcare professionals’ ability to provide healthcare, medical assessment, prescription or related services to you.
